General information and instruction for submitting an Animal Care and Use Protocol Form (ACUF)
The Institutional Animal Care and Use Committee (IACUC) must review and approve all research involving the use of vertebrate animals. This review process is to assure humane treatment and compliance with Federal/State government-mandated animal welfare standards.
The Animal Care and Use Protocol Form ACUF and submission deadlines are available at www.montauc.org. Sufficient lead-time is necessary for the committee to review each protocol; this should be taken into consideration in planning start dates or need approval for funding agencies. Receipt deadlines are without exception. The IACUC is not obligated to conduct a review of late submissions even in light of funding deadlines.
Download forms to your desktop. Complete all sections of the application (indicate N/A in the sections that are not applicable to your protocol). Handwritten protocols or protocols submitted with inadequate information are not accepted for review and will be returned to the applicant. Mark “Yes” or “No” in each applicable question, otherwise form is incomplete.
The investigators must fill out the latest version of the ACUF and submit it to the IACUC office at IACUC@montefiore.org for assignment of a protocol number, pre-review and final review.
The Veterinarian must conduct a review of all applications involving more than minimal pain or distress prior to final submission to the IACUC. In addition, the Attending Veterinarian (cnovotne@montefiore.org) is available for consultation regarding drug doses, routes of administration, anesthesia, tranquilizer selection, surgical procedures, search for alternatives and selection of proper animal models.
When classroom activities are to involve the use of vertebrate animals, the protocol form must be submitted in advance of the semester in which the classroom activity is planned.
Prior to participation in any work with research animals, all scientists (faculty and staff) must complete the MMC-Animal Care Orientation administered by the facility manager with the oversight of the IACUC.
All applications that involve the use of Hazardous Agents required approval by the MMC Biosafety Committee, which can be contacted through the Office of Radiation Safety at 718-920-5012. The related section in the ACUF must be signed by a representative from the Biosafety Committee.
Approval and Expiration: According to Public Health Service policy, protocols are valid for 3 years from the date of initial approval. At the end of the third year they must be submitted novo as new submissions and undergo full review by the IACUC. Protocol approval is granted for a period of ONE YEAR. All approved protocols are required to be renewed annually (Annual Review). The principal investigator (PI) is responsible for ensuring that his/her approval does not lapse.
Forms must be submitted prior to the anniversary of your protocol approval date. It is a violation to conduct experiments under an expired protocol.
Principal Investigators are required to notify the committee of any changes made to any current active protocol. Amendment procedures for an approved protocol are available at www.montauc.org
